Complex regional pain syndrome (CRPS) is defined as a “painful syndrome, which includes regional pain, sensory changes (e.g. allodynia), abnormalities of temperature, abnormal sudomotor activity, edema and an abnormal skin color that occurs after an initiating noxious event such as trauma etc.” CRPS may develop after major trauma, surgery, or minor injury, and progresses with a variable course that ranges from self-limiting, mild symptomatology to chronic disease. Based on the patterns of CRPS, two subtypes of CRPS has been found. The CRPS-I and CRPS-II, differentiated by the absence or presence of peripheral nerve damage, respectively, is demonstrated by electrodiagnostic or physical evidence.

Complex Regional Pain Syndrome Treatment
Currently, the treatment of CRPS is restricted to palliative care and symptom-based interventions such as Non-opioid analgesics (Paracetamol, NSAIDs, Nefopam etc.), Weak opioid analgesics (Tramadol, Codeine/Acetaminophen, Opium/PCM etc.), Strong opioid analgesics, Co-analgesics (Corticosteroids, Antiepileptics, Antidepressants, Tricyclic Antidepressants, SNRIs, Bisphosphonate, IV Bisphosphonate, Calcitonin, Ketamine etc.), Local treatment (Joint infusion of corticosteroids, Lidocaine cream, Lidocaine patch, Capsaicin, NSAIDs etc.) and others (including Physiotherapy, surgery, Psychological interventions etc.).
Currently, Neridronate is the only first line drug that has showed its efficacy in CRPS patients and available in the Italy market. It is the only therapy approved for the treatment of CRPS in Italy (Abiogen Pharma).
Complex Regional Pain Syndrome Market
As per the Classification of Chronic Pain (Merskey and Bogduk 2011) of the IASP and fourth edition of CRPS guidelines, “CRPS-I is a syndrome characterized by a continuing (spontaneous and/or evoked) regional pain that is seemingly disproportionate in time or degree to the usual course of pain after trauma or another lesion. The pain is regional (not in a specific nerve territory or dermatome) and usually has a distal predominance of abnormal sensory, motor, sudomotor, vasomotor/edema and/or trophic findings. The syndrome shows variable progression over time. CRPS-I occurs after any form of trauma, particularly a fracture or soft tissue lesion. CRPS-II occurs after nerve damage.”

Given the psychosocial complexity of CRPS, it is generally agreed that inclusion of pain-focused cognitive behavioral therapy is beneficial as part of standard care for chronic CRPS. Along with this, the mainstay of treatment is physiotherapy (PT) and occupational therapy (OT) in order to improve range of motion and to avoid atrophy and contractures, although, there is no conclusive evidence in favor of any particular approach.
Corticosteroids, analgesics, non-steroidal anti-inflammatory drugs (NSAIDs), COX-2 inhibitors along with free radical scavengers and opioids were used in CRPS for both prophylaxis and rescue which are mostly prescribed in CRPS market. For patients with dystonia, the use of gamma-aminobutyric acid-β (GABA) agonist – oral baclofen is the first line treatment of choice, while intrathecal baclofen should be used only as a last resort.
