      Lifetime Maintenance Rules & Regulations<\/strong><\/p>\n

      1. You must maintain minimum continued care as treatment planned and appointed by LA Dental Experts<\/a>, and our providers.*<\/li>\n
      2. You must maintain continued hygiene care (at least six-month cleaning) appointments.<\/li>\n
      3. Only ONE whitening solution refill will be rewarded to each patient at future recall cleaning appointments (twice annually, typically every 6 months.)<\/li>\n
      4. Lost or destroyed mouth trays will be replaced at a small replacement fee (currently $49 each.)<\/li>\n<\/ol>\n

        * All patients will receive their whitening package upon completion of all necessary dental treatment as determined by the dentist. Minimum gum and teeth health required to undergo professional whitening in order to prevent complications that may arise with unhealthy teeth and gums.<\/em><\/p>\n
